Consents

Students and parents must consent to participate in YVAPE before school administrators can begin enrollment. The consent forms describe the program and detail any risks and benefits of participating in YVAPE.

Students and parents need to provide their school administrator with the signed forms. Then the school administrator will email the signed forms to yvape-support@health.ucsd.edu.

There are different forms depending upon if the student is younger than 18 years old.

If a student is 12 to 17 years old, the following forms must be signed:
Student Assent / Parent Permission Form

If a student is 18 years or older, the following forms must be signed:
Student Consent Form
